On or about 03/23/2013, U.S. Postal Service Catano- DMDU , #100 Avenida Los Conquistadores, Catano PR - Automated Parcel Bundle Sorter (APBS) Conveyor area:  Authorized employees performing servicing on machines were not adequately trained about the hazards and procedures to minimize those hazards. 29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    a.      On or about 03/23/2013, U.S. Postal Service Cataño- DMDU, #100 Avenida Los Conquistadores, Cataño PR - Automated Parcel Bundle Sorter (APBS) Conveyor area:    Employees were exposed to caught by hazards associated moving machine parts while clearing package jams from APBS conveyor area and were not protected from an accidental start up.  The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the servicing work.  As a result, the remaining applicable energy control elements, involving machine isolation, LOTO device application, dissipation of residual energy, and verification of isolation, were not implemented to protect employees from machine servicing hazards.      Note:  Certification and documentation of abatement must be submitted for this citation item as required by 29. CFR 1903.19(c) and (d), respectively.

On or about 01/24/2013, U.S. Postal Service Cataño- DMDU , #100 Avenida Los Conquistadores, Cataño PR - Air Handlers unit / Water pressure tank area:   Employees were using an electrical pump switch & relay system that was energized and not effectively closed, therefore, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact.
