On or about January 26, 2013 and at times prior thereto, the employer did not ensure that adequate cave-in protection was in place prior to an employee entering a trench. An employee was exposed to cave-in hazards while working from an extension ladder placed inside the unprotected trench.

29 CFR 1926.1053(b)(22):  Employees using ladders were carrying objects or loads that could cause them to lose balance and fall.    On or about January 26, 2013 and at times prior thereto, the employer did not ensure that employees entering and exiting trenches by way of extension ladders did so without carrying equipmnet such as but not limited to shovels.  Employees were exposed to losing thier balance and fallling to the base of a trench that was approximately 10 feet deep.
