29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: On or about January 25th, 2013, employees performing work on the 480 volt AC, 400 Amp switchgear were exposed to electrical shock and burns from arc flashes in that when verifying the switchgear de-energized the employees were not protected with electrically rated personal protective equipment such as rubber insulating gloves.
