29 CFR 1926.28(a): Appropriate personal protective equipment was not worn by employee(s) in all operations where there was exposure to hazardous conditions: (a) Wean Hall- 4811 Frew Street, Pittsburgh, PA: On or about January 22, 2013, an employee used a high voltage detector to test an electrical switchgear rated at 4kv without arc-rated clothing on such as but not limited to arc-rated gloves, long-sleeve shirt and pants.
