29 CFR 1910.22(a)(2): Floor(s) of workroom(s) was not maintained in a clean and, so far as possible, a dry condition: a. At the injection molding machines, such as, but not limited to Desma #23, machine oil had run onto the floor which could result in a slip and fall; the floor was not always cleaned in a timely manner or the machine repairs to stop the leaks were not always made in a timely manner. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citation(s).

29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a. At the injection molding machines, such as, but not limited to Rutil 27 and 28, hot machine oil had collected on the mold, dripping onto employees when removing parts resulting in burns; repairs were not always made in a timely manner. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citation(s).
