29 CFR 1910.424(c)(4): Each SCUBA diver did not have a diver-carried reserve breathing-gas supply that consists of a manual reserve (J-valve), or (2) an independent reserve cylinder that has a separate regulator or that is connected to the underwater breathing apparatus.   This violation was observed on January 23, 2013 at the Amber Jack Fish Farm south of Kailua Kona; where SCUBA divers performed work at the 50 foot depth without having a diver-carried reserve breathing gas supply.

This violation was observed on January 23, 2013 at the Amber Jack Fish Farm sounth of Kailua Kona; where washing and toilet facilities were not available on board the vessel exposing the employees to unsanitary conditions.

29 CFR 1918.105(b)(1):  The employer did not provide personal flotation devices (PFDs) for each employee engaged in work in which the employee might have fallen into the water.  This violation was observed on January 23, 2013 at the Amber Jack Fish Farm sounth of Kailua Kona; where employee was working on the edge of the vessel without wearing a Personal Floatation Device, exposing the employee to drowning hazards.
