29 CFR 1926.451(b)(3): The front edge of all platforms was not 14 inches (36 cm) or less from the face of the work, and no guardrail systems were erected along the front edge and/or no personal fall arrest systems were used in accordance with paragraph (g) of this section to protect employees from falling. Note: See exceptions in paragraphs (b)(3)(i) and (ii) of this section: JOBSITE: The front edge of the pump jack working platform, from which an employee was engaged in installing siding was more than 14 inches from the face-of-the-work/wall, and the employee was not protected from the hazard of falling 12 feet 2 inches to the ground by the use of guardrails, fall arrest system, or other equivalent means. On or about 01/25/13, the employee while installing siding fell to the ground through the "gap" and sustained serious injuries from the incident. The TCHE Bagual LLC was previously cited for a violation of an equivalent occupational safety and health standard {29 CFR 1926.451(g)(1)(vii)}, which was contained in OSHA inspection number 315515892, citation number 01, item 003, issued on 09/25/2009, with respect to a workplace located at Lot #34 Hidden Brook Road, Bethel, CT 06801.

JOBSITE: The "mesh screen", installed as a safety net was not appropriately secured along the entire length and extended to both ends of the pump jack working platform, from which an employee was engaged in installing siding at a height of approximately 12 feet 2 inches from the ground.
