29 CFR 1910.106(e)(6)(i): Adequate precautions were not taken to prevent the ignition of flammable vapors: a) Eighth Floor, HPAC 12 and 13 - The controlling employer did not take adequate precautions to prevent the ignition of flammable vapors, in that, they allowed the use of a highly flammable adhesive (having a flash point of -20 degrees Celsius) in a confined space in close proximate to a number of known ignition sources, including, but not limited to, nearby tack welding operations and portable electric work lights which were not intrinsically safe, on or about 11/25/2012. This contractor is the controlling employer on the subject project, and as such is being cited under the Multi-Employer Worksite Policy. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
