29 CFR 1926.404(f)(6):     The path to ground from circuits, equipment, or enclosures was not permanent and continuous:    a) On or about 1/30/13, at the Buffalo Zoo New Entry Complex under construction, 300 Parkside Ave., Buffalo, NY: An employee used an Airless Paint sprayer to paint in the "Gift Shop Area". The paint sprayer was connected to 120-volt ac power via a series of extension cords, the first of which was plugged into a temporary receptacle inside the building. The paint sprayer was plugged into the last extension cord in the series. That extension cord was only a "two wire" cord which lacked both a ground prong and a grounding conductor.    NO ABATEMENT CERTIFICATION REQUIRED
