29 CFR 1926.451(c)(2):     Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates, mud sills or other adequate firm foundation:    a) North side of building - On or about January 31, 2013 employees conducting roofing work were using a tubular welded frame scaffold for access to the roof and to work on the eaves that was not supported by base plates or other firm foundation.

29 CFR 1926.451(e)(1):     When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.    a) Front of building - On or about January 31, 2013, employees were using horizontally laid extension ladders as scaffold planking that were set on the scaffolding in a manner which fouled the climbing frame requiring employees to use the crossbraces as the means of access and egress.

29 CFR 1926.451(g)(1):     Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:    a) East side of building - On or about January 31, 2013, employees were working on multiple sections of tubular welded frame scaffolding that were not fully planked, using an extension ladder in place of standard planking, creating fall hazards from no less than 12.5 feet above grade due to the complete lack of installed guard rails or other fall protection equipment.

29 CFR 1926.501(b)(10):     Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.      a) East side roof area - On or about January 31, 2013, an employee was working on a roof, no less than 12.5 feet above a lower level, and was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.
