29 CFR 1926.403(i)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by cabinets or other forms of enclosures, nor by any of the means listed in subparagraph's (A) through (D) of this paragraph: 11th. Floor Elevator Lobby - A Federal Pacific service distribution paned board had five missing circuit breakers thereby exposing workers to a 120 volt electrical contact hazards, on or about 01/29/2013. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement. This contractor was the Exposing and Controlling employer pursuant to the Occupational Safety & Health Administration's Multi-Employer Work site Policy.

29 CFR 1926.403(i)(1)(ii): Working space about electric equipment operating at 600 volts, nominal, or less was used for storage: 11th Floor Elevator Lobby - Large stacks of roof insulation panels were stored immediately in front of and surrounding a Federal Pacific circuit distribution panel thereby was completely blocking instant access to the electrical equipment, on or about 01/29/2013. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement. This contractor was both the Exposing and Controlling employer under the Occupational Safety & Health Administration's Multi-Employer Work site Policy.
