29 CFR 1910.179(l)(3)(ii): Adjustments shall be maintained to assure correct functioning of the brakes: Facility - On or about January 31, 2013, the employer does not ensure that the bridge brakes on five Uesco 5 ton overhead cranes were operable, operators used the reverse function to stop the cranes. Employees were thereby exposed to struck-by hazards.

29 CFR 1910.179(j)(2): Frequent inspection. The following items shall be inspected for defects at intervals as defined in paragraph (j)(1)(ii) of this section or as specifically indicated, including observation during operation for any defects which might appear between regular inspections. All deficiencies such as listed shall be carefully examined and determination made as to whether they constitute a safety hazard: On or about January 31, 2013, in the facility, cranes were not inspected at frequent intervals as defined. Items not inspected included, but were not iimiited to, functional operating mechanisms,deterioration of hydraulic systems, hooks, hoist chains, and rope reeving. RELIABLE GALVANIZING COMPANY WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCIPATIONAL SAFETY AND HEALTH STANDARD WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 311371918, CITATION 1 ITAM NUMBER 7A, AND WAS CONFIRNED AS A FINAL ORDER ON JULY 23, 2008, WITH RESPECT TO A WORKPLACE LACATED AT 819 WEST 88TH STREET, CHICAGO, ILLINOIS 60620
