29 CFR 1910.23(a)(2): Ladderway floor opening(s) or platform(s) were not guarded by a standard railing with standard toeboard(s) on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person could walk directly into the opening:   (a)  Employee(s) climbing a fixed ladder to gain access to the first level of the dust collector platform located at the west side of the batch plant were exposed to a fall hazard of approximately 12 feet due to no swing gate being provided for the fixed ladder opening at the top of the platform.   (b)  Employee(s) climbing a fixed ladder to gain access to the second level of the dust collector platform located at the west side of the batch plant were exposed to a fall hazard of approximately 14 feet due to no swing gate being provided for the fixed ladder opening at the top of the platform.

29 CFR 1910.146(g)(1): The employer did not provide training so that all employees whose work was regulated by 29 CFR 1910.146 (permit required confined spaces) acquired the understanding, knowledge, and skills necessary for the safe performance of the duties assigned under 29 CFR 1910.146:   Employee(s) performing the duties of entry attendant had not received training over the confined space entry program addressing knowing the hazards the authorized entrant may be exposed to, communicating with the authorized entrant, when to request emergency services,  performing self rescue, performing no duties that might interfere with the entry attendant duties, and etc., thus exposing the authorized entrants to struck-by / caught in between hazards.
