29 CFR 1926.501(b) (13): Each employee engaged in residential construction activities 6 feet or more   above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest   systems, nor was an alternative fall protection plan which meets the requirements of �1926.502(k)   developed and implemented.                                                                                                             This violation occurred on January 30, 2013, at the jobsite located at 6032 Richmond Avenue in Dallas, Texas 75206; where at least two (2) employees performing roofing repair work were not protected from falling to the lower level.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the   ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder   was used to gain access:     This violation occurred on January 30, 2013, at a jobsite located at 6032 Richmond Avenue in Dallas, Texas 75206; where two (2) employees were exposed to fall hazards while using an extension ladder that did not extend at least 3-feet above the upper landing surface.
