29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: a. At the site - employees were exposed to a fall hazard, in that, the use of fall protection was not enforced while operating a Sunbelt Rentals PCO14 450A aerial lift, serial number 358900. The working height was approximately 10'-8". Violation observed on or about 01/31/13.
