29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    At 701 Donnon St, Council Groves in the shop, machinery was not guarded at the point of operation, exposing employees to a struck by and caught in hazard in the following instances:    1) The Millport Mill in the maintenance area had no point of operation guard.  2) The Rockwell/Delta Drill Press in the die cleaning area had no point of operation guard.  3) The Victor Mill in the tooling department had no point of operation guard.  4) The Prodek Mill in the tooling department had no point of operation guard.  5) The RF Drill Press in the tooling department had no point of operation guard.  6) The PYH band saw in the shop was missing a blade guard on the right side.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:    At 701 Donnon St, Council Groves in the shop, two bench grinders had tongue guards, which were not adjusted to at least a 1/4 inch gap, exposing employees to a struck by hazard in the following instances:    1)  Stony Craft Bench Grinder was missing a tongue guard and had a gap of 1 1/2 inch from the wheel to the outer edge of the wheel guard.    2) Clarke Bench Grinder's tongue guard had a gap of 1 1/2 inch from the wheel to the guard.
