29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: (a) 109 Anchorage Rd in Oxford, MS - On or about January 24, 2013, employees were working on scaffold platforms without fall protection. The employee on the forth level did not have a mid rail on the side of his platform. The employee on the fifth level stacking bricks did not have any fall protection.

29 CFR 1926.451(h)(2): Employees working below a scaffold were not protected from falling tools, materials or equipment in accordance with the provisions of paragraphs (h)(2)(i)-(v) of this section: (a) 109 Anchorage Rd in Oxford, MS - On or about January 24, 2013, employees were on a scaffold stacking bricks and nothing was in place to prevent materials from falling on other employees walking next to the scaffold to access the building. The area on the ground was not barricaded and the scaffold platforms did not have toe boards or netting.
