29 CFR 1910.22(d)(1):     On every building or other structure, or part thereof, used for mercantile, business, industrial or storage purposes, the loads approved by the building official were not marked on plates of approved design and securely affixed in a conspicuous place in each space to which they relate:    (a) The mezzanine where old parts such as bumpers were stored, above the employee bathroom and locker room, was not posted with a load rating.    Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.

29 CFR 1910.304(g)(5):     The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective:    (a) Employee break room on the first floor; The grounding pin of the refrigerator plug was missing. Employees were exposed to electrical hazards.    Abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1910.305(j)(1)(i):     Fixtures, lampholders, lamps, rosettes, and receptacles located within 8 feet of the floor had live parts exposed to employee contact:    (a) Employee first floor break room; The receptacle adjacent to the stacked microwaves was broken and live electrical parts were exposed such that employees could accidentally contact them.
