29 CFR 1926.403(i)(2):   Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by cabinets or other forms of enclosures:  (a) 1803 Jackson Ave West in Oxford, MS - On or about January 24, 2013, the masonry saw used to cut bricks had damaged insulation that exposed employees to live parts and were not enclosed in an enclosure.

(a) 1803 Jackson Ave West in Oxford, MS - On or about January 24, 2013, a scaffold platform extended 23.5 inches past the support. The cantilevered portion was used to support another platform/walkway between two scaffolds.

One of the scaffold legs, sitting over the ground (dirt), did not have a mud sill.
