29 CFR 1926.1053(b)(1): A portable ladder was used for access to an upper landing, and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing, and was not secured: (a)Bass Builders @ 208 Sycamore, Johnstown, CO 80534: On and before January 22nd, 2013, the exposing employer did not ensure that a portable ladder, used to access the top plate level of a single-family home under construction, had side rails extending 3 feet above the upper landing, or was secured. The condition exposed a framing employee to uncontrolled fall hazard of approximately 10 feet.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor was an alternative fall protection measure provided. (a)Bass Builders @ 208 Sycamore, Johnstown, CO 80534: On and before January 18th, 2013, the exposing employer did not ensure that fall protection systems were utilized by employees engaged in sheathing operations while working at the unprotected edges on the second story of a single-family home under construction. Employees were not utilizing a fall protective system. Employees were exposed to fall hazard of approximately 20'. (b)Bass Builders @ 208 Sycamore, Johnstown, CO 80534: On and before January 22nd, 2013, the exposing employer did not ensure that fall protection systems were utilized by employees engaged in framing activities while working at the unprotected edges of the top plate located on the second story on a single-family home under construction. An employee was not utilizing a fall protective system. An employees was exposed to fall hazard of approximately 20' Bass Builders was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 442614, citation number 1, item number 1 and was affirmed as a final order on November 28th, 2012 with respect to a workplace located at 1542 Dryland, Loveland, CO 80537. Bass Builders was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 314663618, citation number 1, item number 2 and was affirmed as a final order on April 22nd, 2011 with respect to a workplace located at 2984 Ariel Drive, Loveland, CO 80537.
