29 CFR 1926.452(k) (1): Ladder jack scaffold platforms exceeded a height of 20 feet (6.1 m): a) On or about January 30, 2013; roof of residential home, employees were observed working off or walking on a ladder jack scaffold platform that that was 25 feet from ground level while replacing slate shingles on a 10 in 12 pitch residential roof. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.1053(b) (8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder: a) On or about January 30, 2013; roof of residential home, an unsecured portable ladder was used to access the ladder jack scaffold platform used to performing roofing activities. It was observed that the portable ladder was not secured, nor was there any physical barrier to prevent contact with the base of the ladder. The area around the base of the ladder was used as a staging area.
