29 CFR 1926.651(b)(4):While the excavation is open, underground installations were not protected, supported or removed as necessary to safeguard employees: (a) In front of 104 Spring St Springfield, MA: On or about January 28, 2013, employees excavating with a back hoe in an open excavation struck and subsequently pierced an underground gas line which is not in accordance with this subpart: Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
