29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about January 30, 2013 and at times prior thereto employees were exposed to a fall of approximately 30 feet in height to the ground level below while replacing shingles on a roof with a pitch of 5:12.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: On or about January 30, 2013 employees were exposed to a fall of approximately 30 feet in height to the ground below while using a ladder, that had the side rails approximately 18 inches beyond the landing, to access a roof pitch of 5:12 and install shingles.
