29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest system. On or about January 31, 2013, north side of the commercial construction site, employees were exposed to a fall hazard of approximately 13 feet while engaged in roofing activities on a porch overhang.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed. On or about January 31, 2013, on the north side of the construction site, employees engaged in roofing activities on a commercial building utilized an A-frame ladder, while folded and laying against the parapet wall, to climb over to the building roof.
