29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use. On or about February 1, 2013, on the south side of the construction site, an employee was working from a platform not designed to be used/attached to a SKYTRAC. The employee was working from the platform on an overhead canopy of a commercial building.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about February 1, 2013, on the south side of the construction site, employees were exposed to a fall hazard of approximately 14 feet while engaged in welding and steel decking activities on an overhead canopy of a commercial building.
