(a) Location - Indian Ridge, Westford, MA:    On or about 2/1/13, the employer failed to provide employees with fall protection while materials were being handed-up to the second floor that was more than 6 feet to the lower level.    (b) Location - Indian Ridge, Westford, MA:    On or about 2/1/13, the employer failed to provide guard rail systems at window openings where the bottom of the opening was less than 42 inches in vertical height.
