29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: On or about January 30, 2013 an employee was exposed to a fall of approximately 30 feet in height to the ground below while using a ladder, that had the side rails approximately 18 inches beyond the landing, to access a roof pitch 5:12 and deliver shingles.

29 CFR 1926.1060(b): The employer did not ensure that retraining was provided for each employee as necessary so that the employee maintains the understanding and knowledge: On or about January 30, 2013 an employee was exposed to a fall from a ladder of approximately 30 feet in height to the ground below while using a ladder, that had the side rails approximately 18 inches beyond the landing, to access a roof pitch 5:12 and deliver shingles.
