29 CFR 1910.215(a)(2): Abrasive wheel(s) used on grinding machinery were not provided with safety guard(s) which covered the spindle end, nut, flange projections: a) Jones Manufacturing Co., Inc. on or about February 01, 2013, at the address of 20 Fort Monroe Industrial Pkwy. Monroeville, OH: The employer did not assure the spindle, nut and flange projections on the Jet Bench Grinder were adequately guarded. Employees were exposed to caught-by hazards while using the Jet Bench Grinder to debur aluminum parts.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: a) Jones Manufacturing Co., Inc. on or about February 01, 2013, at the address of 20 Fort Monroe Industrial Pkwy. Monroeville, OH: The employer did not assure that the left side (from operator's position) of the Jet Bench Grinder was equipped with a tongue guard positioned within 1/4 inch of the abrasive wheel's peripheral edge. Employees that operated the grinder to debur parts were exposed to injuries associated breakage of the wheel.
