29 CFR 1926.102(a)(1): Employees shall be provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents. a) On or about 2/1/13, on the east side of the building, the employer allowed an employee to operate a Dewalt Impact Drill to install trim on an awning while not wearing eye or face protection.

29 CFR 1926.451(c)(2)(v): Fork-lifts shall not be used to support scaffold platforms unless the entire platform is attached to the fork and the fork-lift is not moved horizontally while the platform is occupied. a) On or about 2/01/13, on the west side of the building, the employer allowed an employee to work from a pallet elevated by a forklift. The pallet was not securely attached to the forklift and the employee was not wearing fall protection. The employee was approximately 20 feet above the ground.
