29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):  (a) Electrical Shop, NAVAHCS, 500 N US Hwy 89, Prescott, AZ 86313: On or about 15 January 2013, and at times prior thereto, the employer did not initiate and maintain a program for frequent and regular inspections by a competent person of the "live-Line" hotsticks used by employees assigned to perform electrical work above 480v nominal, exposing them to hazards.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  (a) Transformer Pad Mount #37, NAVAHCS, 500 N US Hwy 89, Prescott, AZ 86313:  On or about 15 January 2013, and at times prior thereto, the employer did not instruct the Safety Manager, the Project Engineering Technician, and the Electrician in the recognition and avoidance of the unsafe conditions and the regulations applicable to control or eliminate the exposure to electrical hazards while in the proximity of voltages above 600v nominal when they attempted to start up a transformer which was part of a multi-loop power distribution system enegized at 12470v phase to phase, exposing them to hazards.

29 CFR 1926.954(a): All conductors and equipment were not treated as energized until tested or otherwise determined to be deenergized or until grounded:  (a) Transformers #13, #17, #37, NAVAHCS, 500 N US Hwy 89, Prescott, AZ 86313: On or about 15 January 2013, prior to the attempt to energize the secondary feeds (480v) from Transformer #37, the employer did not test primary or secondary feeder cables supplying that transformer with primary 12470v  phase to phase from adjacent transformers #13 and #17 of the multi-loop system to determine that transformer #37 was completely de-energized, exposing employees in the proximity and within the restricted approach boundary to hazards.
