29 CFR 1910.134(g)(1)(i)(B): Respirators with tight-fitting facepieces were worn by employees with a condition that interfered with the face-to-facepiece seal or valve function    Guam Shipyard: On February 1, 2013, and at times prior thereto, employers did not ensure employees who were exposed to metal fumes were wearing their respiratory protection properly in the following locations;    A. Fabrication Shop: Employees who were exposed to manganese, iron oxide fume, and cadmium were wearing bandanas under their respirators.    B. Big Blue Dry Dock: Employees who were exposed to copper fumes, manganese, zinc oxide, and iron oxide fume were wearing bandanas or balaclavas under their respirators.     This provision is applicable to shipyard employment by 29 CFR 1915.154.

29 CFR 1910.1027(d)(1)(i): The employer did not determine whether any employee was exposed to cadmium at or above the action level:    Guam Shipyard, Fabrication Shop: On February 1, 2013, and at times prior thereto, the employer had not conducted air monitoring to determine employees who were involved in welding, grinding, torch or plasma cutting were exposed to cadmium.  On February 1, 2013, an employee welding on metal from the ballast tank for a total of 345 minutes (sample S1-LL was taken for 180 minutes and S2-LL was taken for 165 minutes) was exposed to an eight hour time weighted average of 0.0006 milligrams per cubic meter of cadmium. The PEL is 0.005 milligrams per cubic meter and zero exposure was assumed for the remaining 135 minutes (in which the employee was not in this area preforming this job task).     This provision is applicable to shipyard employment by 29 CFR 1915.1027.
