The employer failed to ensure that operators have completed a training and evaluation program prior to operating forklifts.

29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification: On or about 01/31/13 at Macon, GA: A surveillance camera was plugged into a receptacle, which was located on a temporary power panel adjacent to the general contractor's trailer. The romex wire that powered the surveillance camera was spliced into the end of an extension cord, and then plugged into the receptacle.
