29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    a)  Production Area - On or about February 6, 2013, employees engaged in servicing and maintenance of Tagging Machines, to include but not limited to removing, cleaning, and replacing the roll cage, for which specific procedures for controlling hazardous energy were not developed.    b)  Small Diameter Room  On or about February 21, 2013, employees engaged in set-up activities of the Torrington Small Tagger, in that they were manually adjusting/checking the tension on the belt and pulley without first unplugging the machine or having specific procedures for the control of potentially hazardous energy developed.   c) Old Mold Room  On or about February 21, 2013,  specific procedures for the control of hazardous energy sources were not developed for employees who are engaged in servicing and maintenance of the Pilger Tube Reducer.

29 CFR 1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by rotating parts:   a)  Production Floor - On or about February 7, 2013, employees performing de-rodding operations at draw benches were not protected from crushing and amputation hazards, in that rotating de-rodding rollers were not guarded.  b)  Small Diameter Tube Shop - On or about February 7, 2013, employees performing de-rodding operations at draw benches were not protected from crushing and amputation hazards, in that rotating de-rodding rollers were not guarded.  c)  Small Diameter Tube Shop - On or about February 7, 2013, employees operating tube drawing benches were not protected from the hazards created by nip points, in that the rotating wheels on the Trolley Plows were not guarded.  d)  Small Diameter Tube Shop - On or about February 7, 2013, employees operating the Eddy Current machine were not protected from the hazards created by nip points, in that the rotating feeding rollers were not guarded.  e)  Old Mold Shop - On or about February 7, 2013, employees using the centerless grinder were not protected from amputation hazards, in that the rotating rollers were not guarded.

29 CFR 1910.151(b): First aid supplies were not readily available:  (a)  Tool Crib - On or about February 6, 2013, where the employer has a first aid kit accessible for employees, first aid supplies were not available.  Abatement Note: ANSI Z308.1-2003 Minimum Requirements for Workplace First Aid Kits. According to the ANSI document, a basic workplace first aid kit should include: - At least one absorbent compress, 32 sq. in. (81.3 sq. cm.) with no side smaller than 4 in. (10 cm)  - At least 16 adhesive bandages, 1 in. x 3 in. (2.5 cm x 7.5 cm)  - One roll of adhesive tape, 5 yd. (457.2 cm) total  - At least ten packets of antiseptic, 0.5g (0.14 fl oz.) applications  - At least six applications of burn treatments, 0.5 g (0.14 fl. oz.)  - Two or more pairs of medical exam gloves (latex or non-latex)  - At least four sterile pads, 3 in. x 3 in. (7.5 x 7.5 cm)  - One triangular bandage, 40 in. x 40 in. x 56 in.
