29 CFR 1926.1428(a):   The employer of the signal person must ensure that each signal person meets the qualification requirements (paragraph (c) of this section) prior to giving any signals.    Jobsite:  The employer of the "employee" assigned to act as the signal person for the crane did not ensure that the "employee" was qualified by meeting the requirements, (c)(1) through (c)(5) of this section prior to assigning the task.

29 CFR 1926.1431(g)(1)(i)(B):  Hooks used in the connection between the hoist line and the personnel platform (including hooks on overhaul ball assemblies, lower load blocks, bridle legs, or other attachment assemblies or components) must be closed and locked when attached.    Jobsite:  The bridal ring of the personnel platform from which two employees were working was attached to the hook of the  headache ball of the crane that had a self-closing but was not a lockable latch.

Jobsite:  The employer did not train each employee, exposed to fall hazards while working from the personnel platform that was hoisted by a crane on all of the appropriate requirements addressing fall protection.

29 CFR 1926.1431(k)(2)(ii):   The employer did not ensure that each employee was not standing, sitting on or working from the top or intermediate rail or toe board, or use any others means/device to raise their working height.    Jobsite:  The did not ensure through visual observation, instruction and training that the employees, iinstalling a cell antenna were not standing on the top rail of the personel platform that was hoisted by a crane approximately 100 feet in the air.

29 CFR 1926.1431(k)(10)(i):   Except over water, employees occupying the personnel platform must be provided and use a personal fall arrest system. The system must be attached to a structural member within the personnel platform. When working over or near water, the requirements of  1926.106 apply.    Jobsite:  The fall protection system that was used by the employees while installing new cell antennas from the personel platform that was hoisted by a crane was not attached to a structural member within the personnel platform.

29 CFR 1926.502(d)(6)(i):   A snaphook not of the locking type was engaged (i) directly to webbing, rope or wire rope; (ii) to each other (iii) to a Dee-ring to which another snaphook or other connector is attached;(iv) to a horizontal lifeline; or (v) to any objectwhich is incompatibly shaped or dimensioned in relation to the snaphook such that unintentional disengagement could occur by the connected object being able to depress the snaphook keeper and release itself.    Jobsite: The snaphooks of the personal fall arrest system that were worn by the employees while installing cell phone antennas  from the suspended personel platform were connected directly to the eye of a single synthetic sling.
