29 CFR 1910.23(c)(3): Regardless of height, open-sided floors, walkways, platforms, or runways above or adjacent to dangerous equipment, pickling or galvanizing tanks, degreasing units, and similar hazards were not guarded with a standard railing and toe board:     (a)  Cardboard Baler Conveyor - Employees push scrap cardboard through a 5 foot wide x 42 inch high opening in the loading dock guardrail onto a moving conveyor, which carries it up an incline into a horizontal baler.  The employees were exposed to the hazards of falling onto the moving conveyor on or about February 6, 2013.             Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

The employer did not provide energy control procedures that specifically outlined the steps for isolating the conveyor systems.  Although the company had written procedures in appendix G of the Lockout Tagout program, the procedures did not include both the specific type(s) and location(s) of energy isolating devices for each piece of equipment, on or about February 6, 2013.      Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

Employees were exposed to electrical shock hazards on or about February 6, 2013.        c) Shipping Department , South Work Station - The employer permitted relocate-able power taps (surge suppressors) to be connected to each other and used to provide power to computers, computer monitors, printers, mail franking machines, and a scale.  Employees were exposed to electrical shock hazards on or about February 6, 2013.        d) Shipping Department - The employer permitted standard grounding connectors to be used at the end of flexible cords that were used as pendent branch circuits. Employees were exposed to electrical shock on or about February 6, 2013.           Abatement certification required within 10 days after abatement date. 29 CFR 1910.305(a)(2)(v)(C): Branch-circuit conductors were laid on the floor:    a) Shipping Department - The employer allowed a branch circuit consisting of a relocatable power tap and flexible cord to be run on the floor from the middle work station, under a rubber mat, below the conveyor to power the computer terminal used for controlling the conveyor system.  Employees walked over and stepped on the branch circuit during the work shift and were exposed to the hazard of electrical shock, on or about 6 February 2013. 29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour:   (a) West Side, Back Of Distribution Center Near The Old Rail Spur - The employer did not ensure that the oxygen cylinder in storage was separated from fuel gas a minimum of 20 feet or by a barrier at least 5 feet high having a fire resistance rating of at least one-half hour.  The oxygen/acetylene torch set was last used the previous week when an employee used to to cut apart a tow motor.  The oxygen cylinder was observed on or about February 6, 2013.   Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
