Section 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards:     (a) On or about January 31, 2013, for the employees who were exposed to crushing hazards where the hook used on the chain to suspended the power tongs did not have a means for the hook to be secured at the Pioneer Drilling Rig by the corner of Highway 1806 and County Road 10, Watford City, ND.     Abatement Note:  Among other methods, one feasible and acceptable method of abatement would be to follow the recommendations of the American Petroleum Institute Recommended Practice 7K, which includes, among other requirements, Section 9.18.5  which specifically covers "Snub-lines for Manual and Power Tongs".

Abatement Note:  Among other methods, one feasible and acceptable method of abatement would be to follow the recommendations of the American Petroleum Institute Recommended Practice 54, which includes, among other requirements, Section 9.9.4 which states, "All tongs should be securely attached to a suitable fixed structure using wire rope or a stiff arm."
