29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification which included the requirements as outlined in 29 CFR 1910.132(d)(2):(a)(LOCATION)(IDENTIFY SPECIFIC OPERATION(S) AND/OR CONDITIONS)(DESCRIBE HAZARD(S) WHERE NECESSARY)     The employer failed to conduct hazard assessments prior to exposing employees to potential electrical hazards associated with employees working on high voltage electrical wiring without wearing the appropriate personnel protective equipment that meets the requirements of  NFPA 70E and required by Subpart I of this part.  This was most recently evident during the inspection of the facility located at 3445 Stolley Park Road, Grand Island, NE 68802, where the employer has an employee wiring in a Miller wire welder into an existing bus box with live exposed parts without requiring that they de-energize and lockout the bus bar or wear the necessary personnel protective equipment to prevent an employee from being injured in the event of an arc flash as demonstrated in this accident.  Instances of personnel protective equipment include but are not limited to:    i.   Employee working on the bus box without wearing arc flash protective jacket or pants to protect  body.  ii.  Employee working on the bus box without wearing arc flash protective hood and helmet to protect face, neck and head.  iii. Employees working on the bus box without wearing arc flash protective gloves to protect hands.

29 CFR 1910.333(b)(2): While an employee was exposed to contact with parts of fixed electric equipment or circuits which were deenergized, the circuits energizing the parts were not locked out or tagged or both in accordance with the requirements of this paragraph (b) of 29 CFR 1910.333(b)(2):     The employer failed to protect employees from potential electrical hazards associated with employees working on high voltage electrical wiring without locking out and or tagging out the live energy source to the equipment they were working on.  This was most recently evident during the inspection of the facility located at 3445 Stolley Park Road, Grand Island, NE 68802, where the employer has an employee wiring in a Miller wire welder into an existing bus box with live exposed parts without requiring that they de-energize and lockout the bus bar.
