29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor was an alternative fall protection measure provided. (a) Roman Construction, Inc. @ 10912 Dayton Way, Henderson, CO 80640: On and before January 22nd, 2013, the exposing employer did not ensure that fall protection systems were utilized by employees engaged in framing activities while working at the unprotected edges of the second story top-plate on a single-family home under construction. Employees were not utilizing a fall protective system. Employees were exposed to fall hazard of approximately 18'.

29 CFR 1926.1053(b)(1): A portable ladder was used for access to an upper landing, and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing, and was not secured: (a)Roman Construction, Inc. @ 10912 Dayton Way, Henderson, CO 80640: On and before January 22nd, 2013, the exposing employer did not ensure that a portable step-ladder, used to access the top plate level of a single-family home under construction, had side rails extending 3 feet above the upper landing, or was secured. The condition exposed framing employees to uncontrolled fall hazard of approximately 10 feet.
