29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a) In the Planer Mill Department at the Sorter - On or about, and at times prior to April 3, 2013, the employer did not ensure employees who worked at heights 4 feet and greater were not provided with fall protection such as, but not limited to personnel fall arrest system, restraint system, or work platform with standard guardrail system attached when accessing the top of the Sorter to clear a jam, thus exposing employees to struck-by and fall hazards up to 15 feet. ABATEMENT CERTIFICATION IS REQUIRED

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed: a) Planer Mill Department Accumulation Chain-On or about, and at times prior to January 31, 2013, the employer did not properly guard rotating sprockets, thus exposing employees to caught-in, and crush-by hazards. b) Planer Mill Department Stick Unscrambler-On or about, and at times prior to January 31, 2013, the employer did not properly guard rotating sprockets, thus exposing employees to caught-in, and crush-by hazards. c) Planer Mill Department Stacker-On or about, and at times prior to April 3, 2013, the employer did not properly guard a rotating sprocket, thus exposing employees to caught-in, and crush-by hazards. ABATEMENT CERTIFICATION IS NOT REQUIRED.

29 CFR 1910.265(c)(4)(iv): Elevated walk(s) and runway(s), 4 feet or more from floor level, were not provided with a standard railing: a) On the Planer Mill Cull Table (platform)- On or about, and at times prior to January 28, 2013, guardrails were not installed on all open sides of the platform, thus exposing employees to fall hazards up to about 9 feet 8 inches. b) At the Planer Mill Cull Table Quality Control area - On or about, and at times prior to January 28, 2013, when large gaps occurred on the accumulation chain adjacent the cull table (platform) employees were exposed to a fall hazard, thus exposing employees to fall hazards up to about 8 feet. c) At the Planer Mill next to the Decline Chain - On or about and at times prior to January 31, 2013, the platform was guardrail was incomplete in that it was missing a midrail, thus exposing employees to fall hazards up to about 6 feet. d) At the Planer Mill Sorter platform - On or about, and at time prior to April 3, 2013, guardrails were not installed on the inside of the platform facing the sorter, thus exposing employees to fall hazards up to about 8 feet. ABATEMENT CERTIFICATION IS NOT REQUIRED
