29 CFR 1926.1423(k)(1): The employer did not train each employee who may be exposed to fall hazards while on, or hoisted by, equipment covered by this subpart on all of the following requirements in this subpart that address fall protection. Jobsite: The employer did not train the operator that set up, and provided for use, the crane basket on the fall hazards associated with lifting personnel.

29 CFR 1926.1431(g)(1)(i)(B): Hooks used in the connection between the hoist line and the personnel platform (including hooks on overhaul ball assemblies, lower load blocks, bridle legs, or other attachment assemblies or components) must be closed and locked when attached. Jobsite: The bridal ring of the personnel platform from which two employees were working was attached to the hook of the headache ball of the crane that had a self-closing but was not a lockable latch.

29 CFR 1926.1431(k)(10)(i): Except over water, employees occupying the personnel platform must be provided and use a personal fall arrest system. The system must be attached to a structural member within the personnel platform. When working over or near water, the requirements of 1926.106 apply. Jobsite: The fall protection system that was used by the employees while installing new cell antennas from the personnel platform that was hoisted by a crane was not attached to a structural member within the personnel platform.
