29 CFR 1910.147(c)(4)(ii):  Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D)(h):     a/ Hexacon/Roselle Pk. NJ Site  Production/Maintenance Areas:  The lockout program provided did not include, but not limited to:    1/ the program was not documented and tailored to the site specific equipment to be worked on   2/ there were no specific steps documented to de-energize the equipment   3/ means to control the use and identity of locking devices  4/ designated personnel involved in the program, etc.      Equipment include but not llimited to: Milling Machines, Lathes, CNC Machines, etc.    Observed on or about 2/4/2013.

29 CFR 1910.335(a)(1)(i):   Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed:     a/  Hexacon/Roselle Pk. NJ Site  Production/Maintenance Areas: Where employees are required (on a limited basis) to perform live electrical testing and/or trouble-shooting on machinery/equipment, protectors including but not limited to approved electrical gloves (ANSI rated 00 or 01 - minimum 500 volts) etc. were not provided for each electrical worker.     Observed on/or about 2/4/2013.
