29 CFR 1926.451(e)(6)(vi): 1926.451(e)(6)(vi): Integral prefabricated scaffold access frames being used for access had a spacing greater than 16 3/4 inches: Jobsite: Front of building. Employees were exposed to falls of up to approximately 40 feet while accessing their work zones within the tubular welded frame scaffold structure by climbing the frames where the horizontal members were not uniformly spaced and were spaced as far as 20 inches apart.
