The outlet box was not designed for use as a pendant style device.  Employees using the outlet were exposed to the hazard of electric shock.

29 CFR 1910.305(a)(2)(ix):  All lamps for general illumination were not protected from accidental contact or breakage by a suitable fixture or lampholder with a guard.     a)  Maintenance Shop wall, 726 Frank Street, Pittsburgh, Pennsylvania, 15227:  Observed on or about February 05, 2013, two fluorescent light fixtures located on the maintenance shop wall were not guarded against accidental contact.  Each light fixture was approximately 79 inches above the shop floor.  Employees working in the Maintenance Shop were exposed to cut-by and electric shock hazards.

The modification was not approved by the manufacturer.  Employees using the forklift to lift pallets of salt with the modified forklift were exposed to struck-by hazards.

29 CFR 1910.244(a)(1)(ii): Rated load(s) of portable jack(s) were not legibly and permanently marked in a prominent location on the jack(s) by casting, stamping, or other suitable means:                                                                          a)  Maintenance Shop, 726 Frank Street, Pittsburgh, Pennsylvania, 15227:  Portable automotive lifting devices were not marked with rated load capacities.  The lack of proper labeling created the potential for employee injuries to occur from the misuse of the bottle jacks and floor jacks.
