Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a. At the site - employees were exposed to an approximate 24 ft fall hazard, in that, the use of fall protection was not enforced. Violation observed on or about 02/05/13.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a. West side of building - employees were exposed to an approximate 24 ft fall hazard, in that, the ladder used to access the roof did not extend above the upper landing surface. Violation observed on or about 02/05/13. b. West side of building - employees were exposed to a 9'-10" fall hazard, in that, the ladder used to gain access to the dump buggy tailgate extended 16" above the landing. Violation observed on or about 02/05/13.
