29 CFR 1910.147(c)(7)(i):   The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees. The training shall include the following:  29 CFR 1910.147(c)(7)(i)(A):   Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.   a. At the workplace, the employer had a lock-out/tag-out program in place.  The program did not establish authorized employee training for all indivduals who applied energy isolation devices on energy sources for equipment used in the workplace, exposing employees to an amputation hazard.

One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.      a. At the workplace, the employer did not ensure a weber machine near the east side of the building was guarded at the ingoing nip point of the paper feeder, exposing employees to a caught-in hazard.

29 CFR 1910.1030(c)(1)(iv):   The Exposure Control Plan shall be reviewed and updated at least annually and whenever necessary to reflect new or modified tasks and procedures which affect occupational exposure and to reflect new or revised employee positions with occupational exposure.     a. At the workplace, the employer had not reviewed the exposure control plan annually.  The last documented review of the exposure control plan was July 28, 2003.  Employees on the first aide team had occupational exposure to blood.
