29 CFR 1910.119(j)(2): The employer did not establish written procedures to maintain the on-going integrity of process equipment. (a) Aryzta LLC - The employer did not ensure the mechanical integrity program procedure listed to contain specific employer procedural steps or instructions to assure inspection frequencies for RB-1 low stage booster compressor and the RC-3 high stage screw compressor. (b) Aryzta LLC. - The employer did not ensure the mechanical integrity program established criteria to determine when inspection testing and preventive maintenance activity frequency must increase. (c ) Aryzta LLC. The employer did not ensure the mechanical integrity program contain specific employer procedural steps or instructions to assure the mechanical integrity of the equipment and detail to provide the employers' inspectors (including contract inspectors) instruction on how to conduct inspections and tests of insulated and non-insulated piping to assure their mechanical integrity. (d) Aryzta LLC. - The employer did not ensure the mechanical integrity program contained specific detail on the specific contractors duties to maintain the ammonia refrigeration system.

29 CFR 1910.119(d)(3)(ii): The employer did not document that the equipment in the process complied with recognized and generally accepted good engineering practices: a) Aryzta LLC - The employer did not document that safety controls, for the ammonia refrigeration system including interlocks and shutdowns, were in compliance with RAGAGEP. The safety controls including but not limited to interlocks and shutdowns were not in compliance with complied with recognized and generally accepted good engineering practices such as IIAR Bulletin 110 for specific safety systems identified as safeguards in the 2011 process hazard analysis (PHA) that include but are not limited to the following:. 1. Emergency Stop (E-Stop outside the Engine Room). 2. High Motor Amps Cutouts. 3. Low Suction Alarms and Cutouts. 4. Draeger Point Guard Alarm. 5. High Discharge Pressure Cutouts. 6. High Discharge Temperature Cutouts. 7. High Oil Pressure Cutouts. 8. Low Oil Pressure Cutouts. 9. High Oil Temperature Cutouts. 10. Low Oil Temperature Cutouts.
