Elyria, OH: The employer failed to maintain fully charged portable fire extinguishers in areas designated by the employer. Several discharged units were kept in the location where employees would access them in the event of a fire. Employees were exposed to fire hazards in that, when necessary no serviceable units would be available for use.

Elyria, OH: The employer failed to ensure the valve cap on a compressed oxygen cylinder was on the tank while in storage/not in use. Employees working in the area were at risk of serious injury in the event the unsupported tank overturned, dislodging the valve assembly and creating a violent release.
