29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level. a. At the site, employees were exposed to fall hazards while gaining access to and from the 4 story building using mast climbers which had deficient railings. The mast climbers were installed at the perimeter of the building and were open sided at the center support, an area of five feet at the wall side where they were tied to the building, at the ends, and at sections where the mast climbers were extended past the building's edge.
