29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): 147 North Main St., South Deerfield, MA On 3/8/13 and at times prior, employees were exposed to moving blades, sharp bits, and power press points of operation on numerous machines. IM Product Grinding Area Instance 1. Grizzly #3 Band Saw had an unguarded area above the guide rollers where employees could access the point of operation. Additionally employee operated a band saw without adjusting the blade guard to just above the material to be cut. Mold Build Room Instance 2. Whitcomb planer with Bridgeport milling head had no point of operation guarding. Instance 3. Bridgeport Mill Drill Press had no point of operation guarding. Instance 4, Victor Engine Lathe Model 1630 Lathe did not have point of operation guarding. Instance 5. Do-All Band Saw #7 had an unguarded area above the guide rollers where employees could access the point of operation. Metal Bending Area Instance 6. Pem machine operators needed to defeat the two hand control safety buttons because there was no fixture to hold the materials. Instance 7. All Steel Brake Press did not have any fixed guards or motion sensing devices. It was operated by two hand control buttons located on a movable pedestal able to be close enough to machine to allow other body parts to be in the danger zone during operation. Deburr Area - Instance 8. Grizzly Bench/Floor Radial Drill Press had no point of operation guarding. Dog Sell Area Instance 9. Chop Saw Machine had a hinged panel that allowed employee access to cutting blade. Instance 10. Full U Dog Driller Machine had a hinged panel that allowed employee access to cutting bits. Instance 11. Standard Dog Driller Machine had a hinged panel that allowed employee access to cutting bits. Machine Shop Area Instance 12. Pem Machine in the machine shop was had an intermittent safety feature. Instance 13. Cincinnati Hypro planer mill with Bridgeport milling head had no point of operation guarding. Instance 14.
