The scaffold as erected had numerous deficiencies including but not limited to missing guardrails on a working level of 13 feet, working levels not fully planked, missing cross braces, cross braces not connected, missing base plates, and improper loading of leg support structure, exposing employees to a fall or collapse hazard at a height of approximately 13 feet, 3 inches above a lower level.
